Options Counseling services are designed to support individuals in making important decisions about their long-term support services. Options Counselors provide one-on-one counseling and decision support about long-term supports and services to residents, their family members, caregivers and significant others. The Options Counselor helps individuals develop a plan that fits their specific needs and encourages the use of important community connections that are essential for a good quality of life.

This service is available at no charge to all older adults and individuals with disabilities regardless of age or income. The LCA Program along with Options Counseling is an exciting new effort to educate residents about their options and empower them to fulfill their desire to transition out of the skilled nursing facility, although transitioning from a facility will not be possible for everyone.
